A field-by-field guide explaining what the DS-160 is asking, which answer applies to your situation, what not to guess, and what to double-check before submitting your application.

Before You Start: Don't Rush

The DS-160 usually takes significant time to complete. Keep your documents beside you and answer according to your real circumstances. The examples on this page are fictional.

The correct answer is the one that accurately describes you. Do not change a truthful answer because another answer sounds stronger. John de Vries and all examples below are fictional.

Your legal identity, names, sex, marital status and place of birth.

Have You Ever Used Other Names?

YES Choose Yes if you have used another legal name, maiden name, previous married name, religious name, professional name, or another name by which you have been known.
NO Choose No only if you have never used another name.
Common mistake Forgetting a previous surname because only part of the name changed.

Telecode Name

A telecode is a four-digit numerical representation used for characters in certain non-Roman alphabets.

YES Select only if you actually have a telecode representation.
NO Select if you do not use or have one.
Do not invent a telecode.

Sex

MALE Select Male if your sex at birth is male.
FEMALE Select Female if your sex at birth is female.

Passport showing X or another marker?

Under current Department of State instructions, the DS-160 sex field should reflect sex at birth.

Marital Status

SINGLE Use when you have never entered a legal marriage or applicable legal relationship.
MARRIED Use when you are currently legally married.
DIVORCED Use when a previous legal marriage formally ended in divorce and you are not currently married.
WIDOWED Use when your spouse died and you have not since remarried.
LEGALLY SEPARATED Use when you remain legally married but have a legally recognized separation.
COMMON LAW MARRIAGE Use only if your relationship legally qualifies as a common-law marriage.
CIVIL UNION / DOMESTIC PARTNERSHIP Use if your legal relationship corresponds to this status.
Living apart from your spouse does not automatically mean you are legally single.

Date of Birth

Enter your date of birth as shown in your passport.

Check day, month and year carefully.

City / State / Province / Country of Birth

Enter the actual place where you were born. Use the current name of the place where requested.

YES Select if you currently hold or previously held another nationality.
NO Select only if the nationality already entered is the only nationality you have ever held.

Permanent Resident of Another Country

This asks whether a country other than your country of nationality has granted you permanent resident status.

YES Select when you legally hold permanent residence in another country.
NO Select when you do not.
A temporary student or work residence permit is not automatically permanent residence.

National Identification Number

Enter the unique national identification number issued to you by your government, where applicable.

If no applicable number exists, use the CEAC Does Not Apply option where available.
Do not enter your passport number merely because both are government numbers.

U.S. Social Security Number

If you have been issued a U.S. SSN Enter that actual U.S. Social Security Number.
If you have never had a U.S. SSN Select Does Not Apply.
Do not enter a Dutch BSN, foreign national ID, passport number or foreign social-security number.

U.S. Taxpayer ID Number

If you have a U.S. taxpayer identification number Enter the number actually issued to you.
If you do not have one Select Does Not Apply.
Do not enter a foreign tax number in a field specifically asking for a U.S. number.

Address and Phone Information

Your current home address, mailing address, telephone numbers, email addresses and social media history.

Home Address

Enter the address where you actually live: street address, city, state/province where applicable, postal code and country.

Use Does Not Apply only when a requested element genuinely does not exist.

Is Your Mailing Address the Same?

YES Select when mail should be sent to the same address where you live.
NO Select when your mailing address is different and enter that address.

Primary Phone Number

Enter the phone number where you are most likely to be reached. It may be a mobile or landline number.

Secondary and Work Phone Numbers

Enter additional current phone numbers where applicable. Use Does Not Apply when CEAC provides that option and the field genuinely does not apply.

Other Phone Numbers Used in the Last Five Years

YES Report additional telephone numbers you have used during the requested period.
NO Select if you have not used any additional numbers during that period.

Email Address

Enter your current email address accurately.

A typo in an email address is easy to make. Check every character.

Other Email Addresses Used in the Last Five Years

YES Provide other email addresses you used during the requested period.
NO Select if no additional email addresses were used.

Social Media Presence

Select each platform requested by the DS-160 that you have used during the relevant period and enter the identifier or handle requested.

The form asks for the social media identifier or handle, not your account password.
Do not invent identifiers or omit applicable accounts simply because they are inactive now.

Check letters, numbers and similar-looking characters carefully.

Passport Book Number

Some countries issue a separate passport book number. If your passport does not contain one, use Does Not Apply where the form allows it.

Do not automatically enter the passport number again.

Issuing Country / Authority and Place of Issue

Enter the country or authority that issued the passport and the requested city, state/province and country information.

Issuance and Expiration Dates

Copy these dates directly from the travel document.

Have You Ever Lost a Passport or Had One Stolen?

YES Select if any passport or travel document was lost or stolen. Provide the requested details.
NO Select only if this has never happened.

Purpose of Trip to the United States

Select the category and visa classification that corresponds to the real purpose of your trip.

Do not choose a visa category because somebody told you it is easier.

Have You Made Specific Travel Plans?

YES Use when you have specific itinerary information. CEAC may request arrival/departure dates, flight information, cities and locations you intend to visit.
NO Use when your itinerary is not yet finalized. You will normally provide an intended arrival date and intended length of stay.
Do not invent airline reservations simply to select Yes.

LENGTH OF STAY: 10 DAYS
These should reflect your genuine current travel plan. There is no magic duration that makes an application stronger.

Address Where You Will Stay in the U.S.

Enter the hotel, residence, school accommodation, friend's home or other address where you genuinely expect to stay.

SELF Select if you are genuinely paying your own travel expenses.

Travel Companions Information

Are Other Persons Traveling With You?

YES Select when another person is actually traveling with you. You may be asked for names and your relationship to them.
NO Select when you are traveling alone.

Traveling as Part of a Group or Organization

If the form asks whether you are traveling as part of a group, answer based on the actual arrangement. A tour group, organized delegation or similar group may qualify.

Do not call ordinary family travel a formal organization unless it actually is one.

Previous U.S. Travel Information

Have You Ever Been in the United States?

YES Provide the requested previous U.S. visit information. The DS-160 may request dates and length of prior stays.
NO Select only if you have never been physically present in the United States.
Have the dates of your recent U.S. trips available before starting the form.

Have You Ever Been Issued a U.S. Visa?

YES Provide the requested previous visa information, which may include issuance date, visa number and whether you are applying for the same type of visa.
NO Select when you have never been issued a U.S. visa.
A previous visa that has expired still counts as a visa previously issued.

Have You Ever Been Refused a U.S. Visa, Refused Admission, or Withdrawn an Application for Admission?

YES Select if any of these events occurred and provide an accurate explanation when the form requests one.
NO Select only when none of these situations has ever occurred.
Do not answer No merely because the refusal happened many years ago.

Has Anyone Ever Filed an Immigrant Petition for You?

YES Select if an immigrant petition has ever been filed for you and provide the requested explanation.
NO Select if no immigrant petition has ever been filed for you.
This question is different from simply having applied for a temporary visitor visa.

Contact Person or Organization in the United States

Enter an individual or organization in the United States connected with your trip and capable of identifying or verifying the purpose of your visit where appropriate.

PERSON Examples may include a relative, friend, business associate, employer or school official.
ORGANIZATION Examples may include a hotel, company, university, conference organizer or other relevant organization.

Relationship to You

Choose the relationship that truthfully describes how the contact or organization is connected to you.

Father's Full Name and Date of Birth

Provide your father's surname, given names and date of birth as accurately as you know them.

Use a Do Not Know option only when you genuinely do not know the requested information.

Is Your Father in the United States?

YES Select if your father is currently in the United States. Additional status information may appear.
NO Select if he is not currently in the United States.

Mother's Full Name and Date of Birth

Provide your mother's surname, given names and date of birth as accurately as you know them.

Is Your Mother in the United States?

YES Select if she is currently in the United States. Additional status questions may appear.
NO Select if she is not currently in the United States.

Immediate Relatives in the United States

This generally covers immediate relatives such as fiancé(e), spouse, children or siblings, excluding parents where the form has already asked about them separately.

YES Provide each applicable relative and the requested relationship/status information.
NO Select only if you do not have applicable immediate relatives in the U.S.

Other Relatives in the United States

YES Select when you have other relatives in the United States.
NO Select when you do not.

This section appears when your marital-status answer requires spouse information.

Spouse's Full Name

Enter your spouse's surname and given names accurately.

Spouse's Date and Place of Birth

Enter the requested date of birth, city and country/region information.

Spouse's Nationality

Select your spouse's actual nationality.

Spouse's Address

If the spouse's address is the same as yours, use the appropriate option. Otherwise provide the actual address requested.

The exact fields change according to the primary occupation you select.

Primary Occupation

Select the category that accurately describes your main current activity, such as employment, study, business, retirement, unemployment or another category presented by CEAC.

Your selection determines which additional fields appear.

Employer / School Name

If employed or studying, enter the full official name of the employer, school or institution requested.

Employer / School Address and Phone

Enter accurate current contact information.

Start Date

Enter when your current employment, study or activity began, according to the form.

Monthly Income

Where CEAC displays this field, enter your actual monthly income in the format/currency requested by the form.

Do not increase or reduce your income because you think another figure looks better.

Briefly Describe Your Duties

Write a short, factual description of what you actually do. Use clear job-related language rather than advertising language.

MANAGE CUSTOMER ACCOUNTS, PREPARE MONTHLY REPORTS, AND COORDINATE CLIENT SUPPORT.

Were You Previously Employed?

YES Enter the requested information for previous employment. Current official guidance specifically refers to previous two employers when previous-employer information is requested.
NO Select when you have no applicable previous employment to report.

Previous Employer Details

Provide employer name, address, telephone, job title, supervisor information where requested, employment dates and duties.

Educational Institutions Attended at Secondary Level or Above

Where this section appears, provide each requested school or institution, its location, course of study and attendance dates.

Do not omit an institution merely because you did not obtain a degree there. Follow exactly what the question asks.

Additional Work / Education / Training Information

Do You Belong to a Clan or Tribe?

Answer according to whether this concept applies to you and provide the requested name if applicable.

Languages You Speak

List the languages you actually speak. Use the Add Another option where available.

Countries / Regions Visited During the Requested Period

List countries or regions you visited during the period requested by CEAC, where this question applies.

Use your passport stamps, travel records and calendar if you are unsure. Do not guess from memory if records are available.

Professional, Social or Charitable Organizations

Answer whether you have belonged to, contributed to, or worked for organizations covered by the question, and provide the requested details when applicable.

Specialized Skills or Training

Answer according to the specific examples and wording shown by CEAC, which may refer to specialized areas such as firearms, explosives, nuclear, biological or chemical experience.

Do not guess at what qualifies.

Read the live CEAC wording carefully. If you genuinely have relevant training, answer truthfully and provide the requested explanation.

Military Service

YES If you have served in the military, provide the country, branch, rank/position, specialty and dates requested by CEAC.
NO Select only when you have never served.

Paramilitary / Rebel / Insurgent / Guerrilla / Vigilante Organizations

Answer exactly according to your actual history. If Yes, provide complete details when CEAC requests them.

Communicable Disease of Public Health Significance

Read the current CEAC definition and answer Yes or No according to your actual medical circumstances.

A Yes answer does not mean you should hide the information. Provide the explanation CEAC requests.

Mental or Physical Disorder Posing a Threat to Safety or Welfare

The question is not simply asking whether you have ever had any medical or mental-health condition. Read the specific live wording and answer according to whether the condition described applies to you.

Drug Abuse or Addiction

Answer the CEAC question truthfully according to your history. If Yes, provide any explanation requested.

Criminal history and related conduct.

Arrest or Conviction for an Offense or Crime

Read the exact CEAC wording carefully. The question may apply even when an arrest did not result in conviction.

Do not decide for yourself that an old event is “too minor to count.”

Answer the question as written and seek qualified legal guidance if you are uncertain how a record applies.

Controlled Substance Law Violations

Answer based on your actual history and provide details when requested.

Prostitution / Commercialized Vice

Answer the live question according to whether the circumstances described apply to your history.

Money Laundering

Answer truthfully and provide explanation if required.

Human Trafficking and Related Activities

The DS-160 contains questions relating to trafficking activity, knowing assistance or association described in the live form. Answer each separate question accurately.

Removal, Deportation or Immigration Proceedings

Answer the separate questions shown by CEAC concerning removal, deportation or related immigration proceedings.

Do not answer based only on whether you personally use the word “deportation.” Use the legal event that actually occurred.

Unlawful Presence / Immigration Violations

Read the specific live CEAC question carefully. If you have had an overstay, status violation or other U.S. immigration issue, do not guess how it should be characterized.

Where displayed, answer each question according to your actual history and the precise circumstances described by CEAC.

Voting in the United States in Violation of Law

Answer truthfully according to your actual history.

Renouncing U.S. Citizenship to Avoid Taxation

This question applies only if the specific circumstances described by CEAC actually occurred.

Security questions require extra care.

A Yes answer does not mean you should change the answer to No. If you are uncertain about the legal meaning of an event, get individualized legal advice before submitting.

Student / Exchange Visitor — F, M and J

These fields appear for applicable student and exchange-visitor applications.

SEVIS ID

Copy the SEVIS ID from your Form I-20 or DS-2019, depending on the visa category.

Copy the number directly from the document rather than typing it from memory.

School / Program Information

Enter the school or program name and address corresponding to the information on your I-20 or DS-2019.

Course of Study / Program Information

Enter the course, major or program information requested by CEAC, using your official student/exchange documentation.

Additional Point of Contact

F, M and J applicants may be asked for additional contact persons in their country of residence who can verify information about them. Follow the exact CEAC instructions about who may or may not be used.

Petition-Based Temporary Worker

Applicable categories may include H, L, O, P, R and other petition-based classifications.

Petition / Application Receipt Number

Enter the USCIS petition or application receipt number requested by CEAC.

Have the relevant Form I-129 approval/petition material beside you.

U.S. Employer Information

Enter the employer name, address and other employment information requested by the visa-specific section.

The information should be consistent with your petition documents.

E Treaty Trader / Treaty Investor Sections

Depending on the E classification and applicant role, CEAC can display extensive additional business information.

Business Profile

The form may request information about the U.S. business, its legal structure, ownership, establishment and operations.

Trade / Finance Information — E-1

E-1 applicants may see detailed questions concerning trade, financial activity and the nature/volume of qualifying trade.

Crew and applicable transit applicants can receive questions about the vessel or airline, position, employer, arrival/departure and other crew-related details.

K applicants can receive additional family, petitioner, fiance, spouse or children-related questions depending on the K classification and prior answers.

Children Information

APPEARS FOR APPLICABLE VISA TYPES

When CEAC displays a children section, provide each child's requested identity and relationship information accurately.

Government / Diplomatic / International Organization Information

Applicable A, G, NATO or similar applicants may receive visa-specific questions related to official position, organization, purpose of travel and assignment.

Why everyone does not see the same DS-160

CEAC uses your visa category and previous answers to decide which additional pages appear. If a section shown in this guide does not appear in your own application, that does not automatically mean anything is wrong.

Upload Photo

Digital Visa Photo

The DS-160 process includes the electronic photo step. Use a recent photo that meets current Department of State visa photo requirements.

Do not assume an ordinary passport-style image automatically meets U.S. digital photo requirements.
If the electronic photo upload fails, follow the embassy/consulate and Department of State instructions for providing an acceptable printed photograph where required.

Sign and Submit

Electronic Signature

By electronically signing the DS-160, you certify the application according to the declarations shown by CEAC. Normally the applicant must electronically sign and submit their own application unless an applicable exception exists.

Do not submit until you have reviewed everything.

Submitting the application is different from merely saving it. Make your final corrections before electronically signing and submitting.

Did Someone Help You Complete the Application?

If CEAC asks whether another person assisted you, answer accurately and provide the preparer/assistant information requested. Receiving help does not mean you should hide the assistance.

AFTER SUBMISSION

Save the Barcode Confirmation Page

After successful submission, save and print or otherwise retain the DS-160 barcode confirmation page according to the instructions for your embassy or consulate.

You do not normally need to print the entire DS-160 application. Keep the barcode confirmation page.

What Happens Next?

Submitting the DS-160 is only one part of the visa process. Follow the instructions of the U.S. embassy or consulate where you will apply regarding fees, interview scheduling and required documents.

Final Rule: Never copy the sample answers.

This guide explains how the questions work. Your DS-160 must reflect your own identity, history, travel plans and circumstances. If a question involves an arrest, immigration violation, visa refusal, removal, security issue or another legally complex fact, do not guess at its legal meaning.
